About the Role
As Clinical Package Lead, you will take an important leadership role within a dedicated care package supporting a vibrant lady with an acquired brain injury in her own home and within the community.
She is outgoing, has a fantastic sense of humour and enjoys a busy social life. She loves going to the pub, shopping, visiting the cinema and theatre, and trying new experiences. The successful candidate will understand that excellent care is about more than meeting clinical needs — it is also about supporting the individual to enjoy life, maintain relationships and remain as independent as possible.
Working closely with the Clinical Lead Nurse, Nurses, Personal Assistants, Support Workers, family and wider healthcare professionals, you will help ensure the package runs safely, consistently and effectively.
Key Responsibilities
- Lead and coordinate the day-to-day delivery of person-centred care.
- Support staff to provide safe, consistent and high-quality care.
- Ensure dignity, choice, independence and wellbeing are at the heart of the package.
- Work closely with the Clinical Lead Nurse to ensure clinical care plans and interventions are followed.
- Monitor care delivery and identify any concerns or areas requiring improvement.
- Support the development, implementation and review of care plans and risk assessments.
- Promote safe medication management and ensure procedures are followed.
- Support effective infection prevention and control practices.
- Liaise with family members and healthcare professionals where appropriate.
- Provide guidance, encouragement and support to members of the care team.
- Monitor documentation and ensure records are accurate and up to date.
- Contribute to safeguarding and the protection of the individual.
- Support the individual to access the community and participate in activities she enjoys.
- Help create a positive team culture based on respect, communication and shared values.
- Ensure care is delivered in line with Brightening Lives policies, procedures and CQC standards.
What We’re Looking For
We are looking for someone who is confident, organised and passionate about providing excellent care. You will ideally have experience in a senior care, supervisory or team-leading role and have experience supporting individuals with complex needs.
You will need:
- Experience within health or social care.
- Previous leadership or supervisory experience.
- Good understanding of person-centred care.
- Excellent communication and organisational skills.
- The confidence to lead and support a dedicated care team.
- A proactive and solutions-focused approach.
- The ability to build positive relationships with the individual, family and professionals.
- A genuine commitment to promoting independence, dignity and choice.
- Flexibility and reliability.
- A positive attitude and good sense of humour.
- A full UK driving licence and access to your own vehicle.
