Join HCRG Care Group as a Healthy Family Practitioner based in Preston and play a vital role in supporting children, young people, and families across Lancashire.
Working as part of the Young People & Healthy Families Service, you’ll collaborate closely with Health Visitors, School Nurses, and wider multidisciplinary teams to deliver high-quality, compassionate care. This is a rewarding community-based role where you’ll empower families, promote child development, and help improve long-term health outcomes.
This is a part-time fixed-term opportunity working 30 hours per week with a planned start date of 01/09/26 – 31/08/27.
What you'll be doing:
In this varied and impactful position, you will:

Deliver key elements of the Healthy Child Programme, including developmental reviews (e.g. 1-year and 2–2.5-year checks)

Support children and families with targeted interventions such as behaviour management and infant feeding
-
Run community clinics and Healthy Family drop-in sessions
-
Work directly with families in community settings, including home visits

Build strong relationships with nurseries and local services

Identify safeguarding concerns and escalate appropriately
-
Encourage service uptake and act as an advocate for families’ needs and voices
-
Maintain accurate records and contribute to service improvement
We’re looking for someone who is:

Passionate about improving outcomes for children and families

Confident working independently in community settings

Knowledgeable about child development and early years milestones

A strong communicator, able to engage effectively with families and professionals

Organised, proactive, and committed to high-quality record keeping
-
Dedicated to safeguarding and delivering safe, inclusive care
-
NNEB qualification or equivalent (e.g. BTEC National Diploma in Nursery Nursing, NVQ Level 3 Childcare & Education, Foundation Degree in Early Years/Education)

Experience in health promotion or public health activities
-
Team working experience
-
Awareness of safeguarding and child protection

Strong understanding of child development and parenting

Flexible approach to meet service needs

Ability to work independently within a multidisciplinary team

Accurate record keeping and ability to maintain high standards of documentation

Ability to contribute to audit and research
-
Competence in creating and implementing family‑centred action plans

Strong written and verbal communication skills
-
IT literacy, including local and web‑based systems
-
Commitment to continuous learning and development
-
Understanding of clinical governance
-
Full UK driving licence and access to a vehicle
