We're looking for a passionate and empathetic Social Prescriber to help people address these challenges and connect with the support, services and opportunities that can make a lasting difference.

As a Social Prescriber, you'll work directly with patients who are experiencing social, emotional or practical challenges that affect their health and wellbeing.
You'll take time to understand what matters most to each person, helping them identify goals and connecting them to local services, community groups and support networks that can help them thrive.
No two days are the same. You may support someone struggling with loneliness, help a patient access housing or financial advice, encourage participation in community activities, or work with healthcare colleagues to create holistic support plans.
Working closely with GPs, multidisciplinary teams and community organisations, you'll play a key role in helping people take greater control of their health and wellbeing.

Supporting patients with non-medical issues that impact their health and quality of life.

Carrying out person-centred assessments and wellbeing conversations.

Developing personalised care and support plans.

Connecting people with community groups, activities and local services.

Working collaboratively with GP practices, Primary Care Networks and multidisciplinary teams.

Supporting people to improve confidence, resilience and independence.

Maintaining accurate records and outcome data.

Promoting social prescribing and community-based approaches to health and wellbeing.

Building strong relationships with local voluntary, community and statutory organisations.
We're looking for someone who is compassionate, proactive and committed to improving outcomes for local people.
You'll have:

Experience working within health, social care, community development, public health or a related setting.

Experience supporting individuals with complex or additional needs.

Experience managing a caseload and prioritising workload effectively.

Strong communication and relationship-building skills.

Excellent organisational and administrative skills.

Good IT skills, including Microsoft Office.

A positive, person-centred and non-judgemental approach.

NVQ Level 3 or equivalent experience.

Training in motivational interviewing, coaching or behaviour change approaches.

Experience working with vulnerable groups and underserved communities.

Knowledge of local community resources and support services.

Experience using EMIS, Elemental or similar systems.

Experience working within primary care or multidisciplinary teams.
