An exciting opportunity has arisen within Shropshire Community Health Trust to appoint an enthusiastic Healthy Child Programme Support Worker to join the Telford Public Health School Nursing Team. The team provides services to children and young people within Telford and Wrekin this post will be based in Telford.
Our forward thinking school nursing team supports the health and wellbeing programmes for all children and young people in schools and the wider community.
We are looking for motivated individuals with experience of working with children, young people and families from 0-19, either from an education or youth background. You should have the ability to work within a skill mixed team, under the delegation of the named Specialist Community Public Health Nurse (SCPHN), to deliver allocated components of the Healthy Child Programme across the 0-19 service.
The post holder will assist the Specialist Community Public Health Nurse (SCPHN) to deliver the 0-19 Healthy Child Programme (HCP) and be involved in maximising the health of children, families and the community. This will be through universal and targeted interventions, health promotion and other activities designed to improve wellbeing and achieve health gain. As part of the role you will be involved in delivering the NCMP National Programme
Driving Licence is essential as you will be required to travel to schools and community venues

You will be required to undertake some administrative duties to support the team, including the inputting of clinical data and maintaining accurate health records.
Your role will require you to work in partnership with schools, partner agencies and providers, to promote and enhance the health and well-being of children and families.
