Job Title: Remote Clinical Pharmacist - Prescriber
Overview:
We are currently seeking a dedicated Clinical Pharmacist (Independent Prescriber, with previous experience of working for a Primary Care Network/ GP Practice) to join our Primary Care Network team on a remote basis (we provide laptop and mobile phone). This position offers an exciting opportunity for individuals with a passion for patient care and medication management.
This role is for 12 hours per week (Monday 8 hours, Tuesday 4 hours). Start time 9.00am.
The maternity cover will be from July to February 2027, in the first instance, with the possibility of this being extended.
You must have extensive knowledge of SystmOne.
Duties:
- Action requests for changes in medication and medication reconciliation
- Review patient medical histories and medication regimens to ensure safe and effective drug therapy
- Undertake medication reviews
- Provide drug information and counselling to patients on proper medication use
- Conduct medication reconciliation and ensure accurate documentation of pharmaceutical care interventions
Qualifications:
- Bachelor of Pharmacy degree or equivalent qualification
- Independent Prescriber/ CPEP
- Registration as a pharmacist with the General Pharmaceutical Council (GPhC)
- Proven experience in a Primary Care setting, with knowledge of EMIS Web and SystmOne
- Strong understanding of anatomy and its application in patient care
- Excellent communication and interpersonal skills
- Ability to work effectively in a multidisciplinary team
- Proficiency in medication administration procedures
