We have an exciting role on offer within our Specialist and Lifelong Services as a Preparing for Adulthood and Transitions Lead.
If you are a qualified social worker with experience of working in children’s or adult services and you are looking for something different then this is for you. We have a new and exciting social work role that has recently been established to improve our offer for young people and their parents/carers, who are preparing for adulthood.
To help us recruit social workers who are committed to working in Middlesbrough we are currently offering a 10% supplement to social work salaries. This additional 10% will be reviewed at the end of March 2027.
You will be working with the Head of Specialist and Lifelong Services to help drive changes to practice in both children’s and adult services to improve service delivery and achieve better outcomes for young people.
You will act as a key link and co-ordinator between Children’s Services, Education, Finance, Commissioning and Adult Social Care to improve outcomes for young people, identifying gaps in provision and barriers to more independent living.
You will work with Children’s Services, Education and Commissioning to identify young people who require transition planning, undertake work with the children’s social worker around assessments where eligibility is unclear or to provide further advice and guidance.
You will provide high-quality, person-centred social work support to young people who are preparing for adulthood where necessary and will assist in the co-ordination and transition of young people from Children’s Services into Adult Social Care. You will support parents/carers to ensure they are aware of processes, eligibility and associated financial assessments and contributions. You may be required to undertake some direct work alongside the case holder and if required you will be responsible for completing Care Act assessments in preparation for attendance at DST CHC meetings with the ICB for young people in their 17th year.
Experience in Preparing for Adulthood, transitional safeguarding, CHC alongside knowledge of the Children’s Act and the Care Act is essential.
As this will entail working alongside partner agencies in children’s social care and education you will need to demonstrate effective and confident multi-agency working and the ability to work autonomously.
