We have an exciting opportunity for a Community Support Worker within Home First South Kerrier Team.
The main purpose for this post is to provide a versatile and responsive service to ensure that opportunities for patients’ reablement are optimised within the community setting. The Reablement workers within each locality will form a part of an Integrated Care Pathway for older patients that involve a range of services to prevent hospital admission and support discharge from hospital.
The post holder will be expected to work with patients in their usual place of care this may also include Residential homes and nursing home. The post holder will be expected to be flexible in working patterns to meet the needs of the service and will be rostered to provide a 8-8, 7 day a week service.
