Come and join our compassionate Community Mental Health Team in Derby City
We are a large, friendly and supportive multi-disciplinary team made up of Doctors, Nurses, Social Workers, Occupational Therapists, Peer Workers and Wellbeing Coaches. Together, we deliver both short-term and long-term interventions as part of a truly collaborative, multi-agency approach.
This role sits within our long-term intervention pathway , with a key focus on completing initial assessments for individuals identified through our Single Point of Access (SPOA). You will play a vital role in supporting people to navigate their journey through services, ensuring they receive the right care at the right time.
You will receive training and supervision to ensure you are confident in all aspects of the role.
Undertake comprehensive initial assessments for individuals entering the long-term intervention pathway
Work alongside Consultant Psychiatrists to provide holistic assessments
Support service users to understand and navigate their care pathways
Work collaboratively within a multi-disciplinary and multi-agency team
Contribute to the delivery of high-quality, person-centred mental health care
Promote recovery-focused and strengths-based approaches in all aspects of care

Attend Derby Wellbeing Team meeting structure – relative to WTE.
Demonstrate post registration experience with a thirst for something different.
Be a role model, offering mentoring and supervision to other staff and students.
You will need to have the ability to transport yourself and service users across a wide geographical area without compromising service needs.
