Role Summary
The Community Support Worker plays a primary role in ensuring we deliver high quality, person centred care for our clients. They will become experienced care staff, familiar with the Care Quality Commission standards and assessment processes providing short-term, community-based support to patients and their unpaid carers.
The Service provides a variety of support to meet the needs of vulnerable people in our community. This may range from private home care, respite care, emergency response and is inclusive of any other contract TuVida may engage with in the future.
Key Responsibilities
You will provide care to clients and their families who have recently been referred to TuVida with home care needs. You will have a client group that gives you a satisfying, meaningful role where you know you are making a difference to people’s lives. You’ll provide ‘hands on’ quality care to men and women living in their own homes promoting choice and independence in your daily interactions. You’ll combine companionship and emotional support with practical help with day-to-day tasks such as personal care, taking medication, preparing meals or shopping. You will also:

Person Centred Care
- Provide personal care and support to service users at the point of assessment where required in order to support the timely commencement of care and support within the home.
- Provide community based care to meet the needs of the organisation. This will include but not exclusively, assistance with activities of daily living, including shopping and light domestic duties when needed, support with medication, prompting increased fluid intake, ensuring hydration and nutrition levels and provision of meals, specialized tasks in accordance with training and care plans.
- Provide practical and emotional support.
- Provide a holistic service to the person with care needs and their family carer(s)
Compliance and Safeguarding
- Work as part of a team, liaising with colleagues, other professionals and involve family members to ensure the best possible outcome for carers and clients
- Work with other colleagues to ensure compliance with relevant regulatory requirements, quality assurance policy and protocols, reporting any incidents or accidents to the appropriate CPOs and Service Manager.
- Implement and maintain best practice safeguarding procedures for adults and children, liaising with the appropriate authorities as required.
- Continuously monitor and regularly evaluate service delivery ensuring best practice at all times, consulting with clients to identify their levels of satisfaction and future needs reporting to relevant team members.
- Escalate any concerns or complaints to the relevant CPOs and Service Manager for investigation and resolution.
Learning & Development
- Participate in and keep up to date with all training (mandatory and professional development) related to care and support planning and carrying out appropriate risk assessments including mental capacity assessments
- To keep up to date with all new legislation covering the provision of regulated care services including new care acts such as Care Act 2014 and regulations including those of the Care Quality Commission and Health & Safety Executive.
- To keep up to date with all mandatory ensuring completion in a timely manner to ensure compliance and on-going ability to perform the role

Training/Qualifications
- Completion of Care Certificate
- Commitment to achieve Level II or above in Health & Social Care
- Level II or above in Health & Social Care
- GCSE English and Maths grade C or above.
Experience
- Experience of working in health and social care.
- Proven experience of medication handling and assessments.
- Experience of lone working.
- Use of an electric care system to document daily notes.
- Experience of supporting end of life services.
- Experience of involvement with safeguarding prevention and management.
Skills & Abilities:
- Good Standard of literacy and numeracy.
- Ability to provide high quality personal care and to recognise and implement through good care planning the individual needs of the service user.
- A good general knowledge and application of IT systems including MS office and Outlook.
- Ability to communicate effectively with a wide audience of stakeholders including service users their carers and families and the local community.
- Ability to travel across the county or a wider area if required to support the needs of the service.
