Overview
The Care Coordinator plays a vital role in ensuring that clients receive comprehensive and personalised care. This position involves creating an effective rota, updating on a daily basis and distributing weekly visit lists. The ideal candidate will possess strong organisational skills and a passion for providing high-quality support to staff and Customers.
Responsibilities
- Working closely with the Care Managers and Director to co-ordinating the rota and care services and ensuring the perfect Carer and client match.
- Liaising with family members and relevant healthcare professionals in relation to client care.
- Assisting in the planning, co-ordinating, and scheduling of visits, ensuring continuity of service at all times.
- Assisting with Carers training activities.
- Following up alerts to ensure a timely service.
- Assisting with Carers recruitment.
- Dealing promptly and appropriately with any queries received, referring to the relevant person or team.
- Building and maintaining effective communication with all clients, relatives, and associated care professionals
- Supporting local community and business events and activities.
- Ensuring compliance and quality assurance standards are met in line with company procedures and CQC regulations.
- Reporting any recruitment requirements to ensure client needs are fulfilled.
- Be part of the on call rota , every other weekend.
Qualifications
- Previous experience in an office environment is preferred, demonstrating strong administrative skills.
- Excellent verbal and written communication skills, with the ability to convey complex information clearly.
- Strong organisational abilities with attention to detail to manage multiple cases effectively.
- Empathy and compassion towards clients, with a commitment to providing high-quality care.
- Proficiency in using office software and electronic health record systems is advantageous.
- Ability to work collaboratively within a team while also being self-motivated.
