Registered Learning Disability Nurses – Complex Care (Hampshire)
£38 per hour (Umbrella)
Weekday/Weekend/Days/Nights shifts available

We are seeking a compassionate and reliable Registered Learning Disability Nurse to support children and adults in around the Hampshire. Your care will enable them to live safely, confidently and with dignity in their own home.

About the Client
Our clients range from children to adults who have mental health challenges in their homes. They benefit from structured routines, reassurance and support both at home and in the community. An LDN who is calm, patient and confident working independently will thrive in this role. As per the service user’s location, you will need to be able to drive to attend.
Clinical Requirements
Comprehensive assessment: Assess physical health, mental health, communication needs, behaviour, and social circumstances to develop individualized care plans.
Clinical care: Provide and oversee complex nursing interventions such as:
Medication administration and management.
Enteral feeding (e.g., PEG tube care).
Tracheostomy care (where trained and competent).
Seizure management and epilepsy care.
Stoma and catheter care.
Wound management.
Monitoring long-term conditions such as diabetes or respiratory disease.
Positive Behaviour Support (PBS): Assess behaviours of concern, implement behaviour support plans, and use de-escalation techniques to reduce restrictive practices.
Care planning and coordination: Develop, review, and evaluate person-centred care plans in collaboration with the individual, families, and the multidisciplinary team.
Health promotion: Support healthy lifestyles by promoting nutrition, exercise, vaccinations, screening programmes, and preventive healthcare.
Safeguarding: Recognize and report signs of abuse or neglect, ensuring people are protected and their rights are upheld.
Communication: Adapt communication using accessible formats, visual aids, or alternative communication methods to ensure informed decision-making.
Family and carer support: Educate and support families and carers in managing complex health needs safely.
Multidisciplinary working: Collaborate with professionals such as GPs, psychiatrists, psychologists, occupational therapists, physiotherapists, speech and language therapists, and social workers.
Risk assessment: Identify and manage risks related to health, mobility, nutrition, behaviour, and the environment.
Leadership and supervision: Supervise healthcare assistants and support workers, provide clinical guidance, and ensure high standards of care.
Documentation: Maintain accurate clinical records, complete risk assessments, incident reports, and care documentation in line with legal and professional standards.
Advocacy: Promote equality, dignity, independence, and the rights of people with learning disabilities, ensuring reasonable adjustments are made to healthcare services.
Emergency response: Recognize deterioration, provide immediate nursing interventions, and coordinate emergency care when required.

- Weekend day shifts
- Weekend night shifts
- 12-hour shifts or shorter calls
- Driver requirement if applicable
- Regular pattern (e.g., every Saturday and Sunday)
What the Role Involves
- Delivering compassionate 1:1 care in the client’s home
- Supporting with community access and social activities
- Promoting independence and dignity in all tasks
- Providing emotional reassurance and wellbeing support
- Communicating effectively with the clinical, care and coordination teams
Your presence will help the client feel safe, supported and in control of their daily routine.
