Role Introduction
Are you a skilled leader who aspires to a rewarding role transforming care pathways? We are looking for an experienced Project Manager, with an interest in system working, to lead work with partners across the Leeds Health & Care system to deliver our local ambition to develop a Neighbourhood Health approach.
You would lead delivery of key workstreams in the Active Recovery 2 & Stroke Pathway Programme. This programme of work includes the expansion and strengthening of the existing Active Recovery model and a project to implement an early supported discharge pathway for survivors of stroke. On occasion you may also provide support to a programme testing a new model of integrated, community-based specialist diabetes care.
We welcome applications on a secondment basis from anyone applying from within the Leeds Health & Care Partnership. Roles are offered on a fixed-term basis until the end of March 2027 and may be subject to further extension in 2027/28.
We are open to applications from people looking to work between 22.5 and 37.5 hours per week (0.6-1.0wte)
You will work on behalf of the Leeds Health & Care system, hosted by the Leeds Community Healthcare Business, Change & Improvement Service - a network of skilled and experienced project managers, change agents and business managers. You will work alongside a programme manager and project support officer as part of a smaller team.

We are looking for someone who puts the human touch into project and change management – applying flexibility, empathy and a focus on outcomes to ensure that services are supported to improve and to improve and adapt across organisational boundaries. You will have:
- Experience of confidently leading workstreams within projects
- The ability to use strong interpersonal, communication and relationship management skills with clinical services, corporate services, senior managers and other stakeholders
- The ability to listen, interpret, analyse and present complex information for a range of audiences
- An ability to influence and negotiate to support change processes
- Strong critical thinking and problem-solving skills
- A range of quality improvement skills such as facilitation, process mapping and other methodologies
- The ability to inspire and motivate project teams and services to continuously improve
- High resilience and the ability to manage multiple workstreams at the same time.
Knowledge and understanding of local intermediate care rehabilitation pathways, including the stroke pathway, is what will set you apart as the best candidate to lead this critical work for the local area.
