Detailed job description and main responsibilities
The post holder will be responsible for managing their own workload, prioritising tasks effectively, and ensuring deadlines are met. They will be expected to work independently as well as part of a team, demonstrating flexibility and a commitment to high standards. The role may involve working across multiple sites and requires excellent communication and organisational skills.
Using the Trusts Cancer Management System, the postholder will track and monitor patients progressing them through their cancer pathway effectively and in line with both local and CWT guidance policies escalating any issues causing delays to patient pathways effectively and in a timely manner to seek resolution to the problem .
It is essential that all records are up to date and accurate which involves using multiple Trust systems to identify any information relevant to the patient and their cancer pathway working collaboratively with Op's, Clinical and Supporting Teams to obtain information and clarify plans for diagnostics and seek to expedite where appropriate all investigations , appointments to progress patients through to discharge or diagnosis.
Daily use of generic Cancer Team email boxes to update all relevant cancer PTL information received and to communicate requests for updates and escalations to relevant Teams both internal and external.
Adhere to processes that identify patients with a new diagnosis of cancer needing discussion at MDT to decide on treatment plans and management and highlight these to MDT Coordinators. with full up to date information required for these discussions.
To ensure all diagnostics, appointments procedures requested are undertaken and interpret outcomes from these and effectively request clinical review for next steps.
