About the Role
We are looking for an experienced, caring and reliable Domiciliary Care and Reablement Care Coordinator to join our friendly team.
The successful candidate will coordinate safe, responsive and high-quality domiciliary and reablement services. You will work between our Gloucester office and the community, supporting clients, care staff and healthcare professionals.
During the first three months, you will complete a structured training and probation period at £15.00 per hour. Following successful completion of both your training and probation, your hourly rate will increase to £16.25.
Key Responsibilities
- Coordinate the delivery of safe, reliable and person-centred domiciliary and reablement care.
- Support clients to achieve agreed reablement outcomes and regain their independence.
- Coordinate new care packages and hospital-discharge referrals.
- Manage care staff rotas and respond to absences, changes and urgent cover requirements.
- Match appropriately skilled care staff with clients according to their needs.
- Conduct client assessments, reviews, risk assessments and staff spot checks.
- Create, review and update personalised care and reablement plans.
- Monitor clients’ progress and ensure agreed goals and outcomes are accurately recorded.
- Liaise with clients, relatives, care staff, social workers, NHS professionals and local-authority teams.
- Escalate changes in clients’ needs, safeguarding concerns and risks promptly.
- Mentor, supervise and provide practical support to new and existing care staff.
- Support staff inductions, shadowing and competency assessments.
- Maintain accurate, confidential and up-to-date records on the company’s care-management system.
- Participate in the on-call rota.
- Provide hands-on care in the community when operationally required.
- Help ensure the service complies with company policies and relevant regulatory requirements.
Essential Requirements
- At least one year’s experience as a Senior Care Assistant within domiciliary care or reablement.
- Experience coordinating, supervising or supporting community-based care services.
- A valid UK driving licence and access to your own car.
- Ability to travel throughout the designated service areas.
- Availability to work full-time, Monday to Friday and every other weekend, between 7:00am and 10:00pm according to the rota.
- Ability to participate in the on-call rota.
- Strong communication, organisation and problem-solving skills.
- Ability to work effectively in a fast-paced care environment.
- Willingness to work both in the office and within the community.
Desirable
- Level 2 or above in Health and Social Care.
- Previous reablement or hospital-discharge experience.
- Experience coordinating new care packages and managing staff rotas.
- Experience conducting assessments, client reviews and spot checks.
- Knowledge of safeguarding, medication management and care-quality requirements.

Office-based duties will be completed at our Gloucester office.
