We are looking for a dedicated and ambitious Personal Advisor to join our Leaving Care Service, supporting young people aged 16-25 as they transition from care to independence and adulthood.

This is an exciting opportunity to make a genuine difference to the lives of young people by building trusted relationships, offering practical support and helping them navigate some of the most important stages of their lives.
As a Personal Advisor, you will work alongside young people to help them prepare for adulthood and develop the skills, confidence and resilience needed to live independently.
You will have responsibility for maintaining meaningful relationships with young people, coordinating support plans and advocating for their needs across a range of services including housing, education, employment, training, health and wellbeing.
This is a varied and rewarding role where no two days are the same. You will provide both practical and emotional support whilst ensuring that young people remain at the centre of all planning and decision-making.
Key Responsibilities: -

Build positive, trusting and professional relationships with care experienced young people.

Develop, review and monitor Pathway Plans that reflect young people's aspirations, strengths and support needs.

Provide advice, guidance and practical support regarding housing, finances, education, employment and independent living.

Support young people to access and engage with health services and community resources.

Act as an advocate, ensuring young people's voices are heard and their rights promoted.

Work collaboratively with social workers, housing providers, education services, health professionals and partner agencies.

Maintain accurate and timely case records and contribute to statutory reporting requirements.

Support young people in crisis and respond effectively to safeguarding concerns.

Promote independence while maintaining a strong focus on wellbeing and positive outcomes.
We are seeking an individual who: -

Has experience of working with vulnerable young people or care experienced young adults.
-
Understands the challenges faced by young people transitioning to independence.
-
Has excellent communication and relationship-building skills.
-
Is resilient, empathetic and able to work creatively to overcome barriers.
-
Can balance support and challenge to achieve positive outcomes.
-
Has strong organisational and recording skills.
-
Is committed to safeguarding and promoting the welfare of children and young people.

Works effectively as part of a multi-agency team.
Essential Requirements: -
-
Experience of working directly with young people, families or vulnerable adults.

Knowledge of safeguarding principles and legislation.
-
Ability to manage a caseload and prioritise competing demands.

Full UK driving licence and access to a vehicle for work purposes
