Empowering U Telford are looking for a experienced care coordinator
You will need an understanding the care industry and some knowledge of access people planner, you will be working in a busy office environment working with the team and registered manager, you will be completing care plan review, updating care plans risk assessments, attending MDT meeting and being the point of contact for your nominated service users and team of carers, you will be responsible for completing interviews to ensure we have the correct match between carer and service user , you’ll be key to our success. Clear communication is vital, and a good understanding of IT
You will be required to complete on call on a rota basis
What you will need
Experience as a Care Coordinator within in a busy environment as well as a good knowledge of working in an office delivering care, as well as strong IT skills.
Full driving licence with a good knowledge of the local area.
Combining a highly organised, systematic approach, with a knack for problem solving.

Salary: £28,000
Hours: Monday - Thursday office 08:30-17:00 and Friday 08:30-16:00 and shift cover when required outside these hours, plus paid on call
