We are looking for an experienced and values‑driven Hospital Team Manager to lead our Local Authority hospital social work team. We are an integrated health and social care team working in a multi disciplinary way with occupational therapists and discharge nurses within the team . Based at North Tyneside General Hospital, Rake Lane, North Shields.
This role is critical in ensuring timely, strengths‑based assessments and safe, well‑planned discharges for residents, while supporting patient flow across the health and care system.
You will manage a busy team working within acute hospital settings, delivering statutory duties under the Care Act 2014, Mental Capacity Act, and associated legislation. Working closely with NHS colleagues and partner agencies, you will ensure high standards of practice, effective risk management, and positive outcomes for people and carers.
We are looking for someone who can lead, manage, and develop a hospital‑based adult social care team. Ensure statutory responsibilities are met in line with the Care Act, MCA, and safeguarding duties. Provide professional supervision, support, and performance management for staff. Oversee complex casework, providing guidance on risk, safeguarding, and legal frameworks. Ensure timely assessments, discharge planning, and effective use of community resources. Manage service demand, workloads, and staffing to support hospital flow. Work collaboratively with NHS partners, voluntary sector, and internal council teams. Monitor performance, quality, and outcomes, addressing pressures and risks proactively. Contribute to service development, transformation, and system‑wide improvement initiatives

You must hold a recognised qualification registered with Social Work England, HCPC, or NMC.
