- Ensure accurate records are maintained and share all relevant information to ensure the best outcomes for people.
- Contribute to updating care plans, risk assessments and review reports.

- Promote lifestyles that reflect the wishes and choices of the people we support.
- Develop and ensure you understand each person’s needs including any specialist support they require i.e. communication, eating and drinking guidelines, epilepsy management etc.
- Contribute to the safe and efficient running of premises, having a clear understanding of health and safety and using your knowledge of the person to inform risk assessments and management plans.
- Ensure that care support plans and risk assessments are read, understood and used to guide practice.
- Understand the role of Care Management and liaise with the team as necessary.
- Participate in rotas to meet the needs of the service to ensure the service is appropriately staffed according to commissioning requirements.
- Be accountable for supported people’s finances as relevant to your service area and ensure accurate records are maintained.
- Be flexible to support the needs of the people we support in every area of their lives.
- Order, stock take and manage medication were delegated from your manager and in line with the needs of the service.
- Complete all necessary documentation and records including CAir.
- Understand the Mental Capacity Act 2005 and Deprivation of Liberty and how this impacts on your role.
Essential Criteria:
- Able to use a computer
- Able to develop and maintain supportive relationships with supported people that promote choice and independence
- Able to meet the personal care needs of people within an agreed support plan
- Able to support people with complex needs living in their own homes to access a range of community resources and activities
