Responsibilities:
- Review and interpret physician orders to ensure accuracy and appropriateness of medication therapy
- Collaborate with healthcare professionals to develop and implement patient care plans
- Monitor patient responses to medication therapy and adjust treatment as necessary
- Provide medication counselling to patients regarding proper use, potential side effects, and drug interactions
- Maintain accurate patient records and documentation of medication therapy
Qualifications:
- Doctor of Pharmacy (Pharm.D.) degree from an accredited college of pharmacy
- Strong knowledge of medication administration, including dosage calculations and compounding techniques
- In-depth understanding of human anatomy and physiology
- Excellent communication and interpersonal skills for effective patient care
