Provide support and assistance to a young man with a visual impairment and executive functioning difficulties as the result of a traumatic brain injury.
Shifts will included working alternate weekends and one evening per week as well day time hours.
To work as an advocate and personal assistant to the client and as part of a team within client’s own home or community setting in accordance with their support and rehabilitation plan.
To meet the client’s needs by facilitating rehabilitation, providing companionship, support, advice and supervision to maximise his independence and reduce risks.
To provide a flexible and supportive service which facilitates engagement in activities of daily living and enables the client to reach their full potential and enhances quality of life.
Key responsibilities:
Responsible for implementing a weekly plan as identified by client, family, case manager and/or therapy team for the client, including an activity programme incorporating domestic, recreational, vocational and leisure activities as appropriate.
To support the client in spending quality time with his young son and fulfilling his parenting responsibilities, assisting with child related tasks as required.
To assist client with managing household paperwork and weekly budgeting.
To provide transportation for day to day activities and for social events/days out using client’s Motability vehicle. This includes picking up/dropping off his son for visits/time at client house as required.
As appropriate, assist in the cleaning and maintenance of the home and maintaining a safe environment for the client and other family members. You will report any hazards, defects or deterioration in the condition of the working environment to the case manager as necessary.
To prompt, guide and provide feedback to the client if his difficulties hamper his ability to engage in activities.
Demonstrating initiative and ability to use resources and strategies and adapt client environment when necessary to support the client to maximise his potential.
Keep daily records documenting support provided - communicating any areas of concern clearly and promptly, ensuring all records and documentation are accurate and maintained in accordance with GDPR legislation.
To communicate effectively and work with others (family, case manager, therapy team) as part of a team and in all aspects of the role.
To ensure confidentiality of information in respect of records maintained and tasks undertaken within the client’s home. This includes maintaining strict confidentiality in relation to any personal information (including that of Client’s name, his family and other employees) that may become known to you in the course of your work or associated activities.
To take part actively in supervision with your line manager, Supervision will address professional, clinical, and personal development.
To attend, participate in, and contribute to training courses (internal and external) and team meetings in order to contribute to quality services and self-development
Pay: £14.00-£15.50 per hour
