In recent years, the importance of strong, integrated partnership working across health, social care and housing has become increasingly critical. Delivering effective, joined up services through the BCF, the joint equipment service and Complex Care pathways depends on close collaboration across the systems. Through these partnerships, we aim to ensure that care is coordinated, responsive, and centred on the needs of some of our most vulnerable residents.
As financial pressures continue and models of care evolve, there is a growing need to strengthen system-wide working with health partners and the wider provider market. This role will support the development of integrated, pathway-led approaches that promote prevention, reduce hospital admissions, support timely discharge, and improve long-term outcomes. A strong focus on quality, safety, and person-centred care will underpin all aspects of this work.
We are seeking an individual with a strong blend of strategic thinking, commercial awareness, and relationship-building skills. The successful candidate will play a key role in building and sustaining effective partnerships across the health, housing and care system, aligning priorities, and supporting the delivery of integrated services across BCF, the joint equipment service and Complex Care pathways to achieve shared outcomes.
You will be based in our offices in Winchester 1 day a week on a specified day, working in a hybrid way the rest of the time; including potential travel across the County to visit providers if required.
