We are looking for a motivated and patient-focused Pharmacy Technician to join our friendly and forward-thinking Primary Care Network (PCN) team.
This is an exciting opportunity to support the safe, effective and cost-efficient use of medicines while working as an integral member of our multidisciplinary team. You will play a key role in medicines optimisation, repeat prescribing, medicines reconciliation, prescribing safety and patient-facing support, helping to improve health outcomes across our practice population.
You'll work closely with GPs, Clinical Pharmacists, nurses, community pharmacies and the wider healthcare team to deliver high-quality care and contribute to service improvements that benefit our patients.
What You'll Be Doing
As our Pharmacy Technician, you will:
- Support medicines optimisation and safe prescribing across the practice.
- Undertake medicines reconciliation following hospital discharge and transfers of care.
- Assist with repeat prescribing processes and medication record maintenance.
- Respond to medication-related queries from patients and carers.
- Support prescribing safety audits, quality improvement projects and clinical governance activities.
- Monitor and support the safe use of high-risk medicines in line with practice protocols.
- Lead on identifying and supporting patients with hypertension and raised blood pressure, working within agreed competencies and clinical protocols.
- Promote medication adherence and provide lifestyle advice to support cardiovascular health.
- Support QOF, enhanced services and prescribing incentive schemes.
- Work collaboratively with community pharmacies, secondary care providers and PCN colleagues to improve continuity of care.
- Maintain accurate clinical records and contribute to data collection, audits and reporting.
About You
We're looking for someone who is:
- Registered with the General Pharmaceutical Council (GPhC).
- Passionate about improving patient care through safe and effective medicines management.
- Organised, proactive and able to work independently as well as part of a multidisciplinary team.
- An excellent communicator with strong attention to detail.
- Committed to continuous learning, quality improvement and delivering excellent patient care.
Essential Requirements
- Registered Pharmacy Technician with the General Pharmaceutical Council (GPhC).
- Meets the GPhC qualification requirements for Pharmacy Technicians.
- Enrolled on, undertaking or completed an approved Primary Care Pharmacy training pathway (such as the Primary Care Pharmacy Education Pathway or equivalent).
- Experience working as a qualified Pharmacy Technician.
- Experience supporting medicines optimisation and handling medication-related queries.
- Evidence of continuing professional development (CPD).
Desirable
- Experience working within General Practice, Primary Care or a Primary Care Network.
- Experience undertaking medicines reconciliation.
- Experience supporting QOF, enhanced services and quality improvement initiatives.
- Knowledge of hypertension management and cardiovascular disease prevention.
- Experience supporting care homes and multidisciplinary teams.
