We're looking for an experienced, proactive and compassionate Team Leader to join our leadership team. This is an exciting opportunity for someone who enjoys taking ownership, supporting colleagues and ensuring every shift runs smoothly while maintaining the highest standards of care.
About the Role
As Team Leader, you'll bridge the gap between the management team and frontline staff, taking operational responsibility for each shift while continuing to provide hands-on support to the people who live at Rafael Home.
You'll lead by example, ensuring our values are reflected in everything you do and that every person receives the outstanding care they deserve.
Your responsibilities will include:
- Leading and coordinating the day-to-day running of each shift.
- Supporting, motivating and guiding Senior Support Workers and Support Workers.
- Delivering outstanding, person-centred care tailored to each individual's needs.
- Ensuring care plans, documentation and shift records are completed accurately.
- Conducting effective handovers and communicating important updates to the management team.
- Monitoring the wellbeing of the people we support and responding promptly to any changes.
- Administering medication safely and in accordance with policies and legislation.
- Maintaining a safe, welcoming and well-organised home environment.
- Promoting safeguarding, dignity, independence and choice at all times.
- Building positive relationships with families, healthcare professionals and external agencies.
- Supporting the induction and ongoing development of new team members.
What We're Looking For
We're looking for someone who combines excellent care experience with strong leadership skills.
You'll ideally have:
- Previous experience working with adults with learning disabilities.
- An Enhanced DBS check with Adults' Barred List clearance
- Experience supervising or leading a care team.
- A Level 3 Diploma in Health & Social Care (or equivalent), or be willing to work towards it.
- Excellent communication, organisation and problem-solving skills.
- Confidence in leading shifts and making informed decisions.
- A calm, positive and proactive approach.
- Strong record keeping and IT skills.
- A genuine commitment to delivering exceptional person-centred care.
- A passion for developing others and leading by example.
Working Pattern
This is a permanent position working a flexible shift rota, including weekends and bank holidays.
Flexibility is essential, as the role includes weekend working and participation in the rota to meet the needs of the service.

Pay: From £13.00 per hour
Work Location: In person
