Care Home AssistantPractitioner (CHAPS) Days. Ribble Court Care Home
Hourly Rate: 16.00per hour
Shift: 3-4 days per week, 8am-8pm
Contracted Hours: 38.5 hours per week
We are looking for an experienced Care Home Advanced Practitioner (CHAPS) to join our team at Ribble Court Care Home.
Working alongside our Registered Nurses, you will play a key role in delivering high-quality, person-centred care, supporting clinical interventions, administering medication, and providing guidance and supervision to care colleagues. This is an excellent opportunity for an experienced Senior Care Assistant or Team Leader looking to further develop their clinical skills within a specialist nursing and dementia care environment.
Key Responsibilities
Deliver exceptional, compassionate care that promotes dignity, choice, and independence.
Support residents with a range of clinical interventions under the direction of registered nurses.
Undertake and record clinical observations and monitoring.
Assist with wound care, PEG checks, blood glucose monitoring, catheter care, and stoma care.
Safely administer medication and maintain accurate records.
Support and supervise care colleagues, promoting best practice and high standards of care.
Contribute to care planning, reviews, handovers, and multidisciplinary communication.
Build positive relationships with residents, relatives, and healthcare professionals.
Help maintain a safe, welcoming, and responsive environment throughout the day.
What We Are Looking For:
We're looking for a caring, confident, and motivated individual who can lead by example and provide excellent support to both residents and colleagues.
You will ideally have:
NVQ Level 3 in Health & Social Care (or above).
Previous experience working in elderly care or nursing home settings.
Experience in a senior or supervisory care role.
Knowledge or qualification in the Safe Handling of Medicines.
Excellent communication and teamwork skills.
A commitment to delivering outstanding person-centred care.
Willingness to undertake further training and professional development..
