The Role
You will provide a broad range of Practice Nursing and chronic disease management services, working both autonomously and collaboratively with the wider clinical team.
The practice is keen to support nurses in developing specialist interests and expanding their clinical skills where appropriate.
Key Responsibilities
- Deliver high-quality Practice Nursing services
- Carry out chronic disease reviews
- Manage patients with diabetes, asthma and COPD
- Hypertension and cardiovascular disease management
- Perform health checks and routine clinical assessments
- Administer adult and childhood immunisations
- Provide cervical screening
- Undertake wound care and dressings
- Perform ECGs and routine observations
- Support new patient health checks
- Provide NHS health checks
- Deliver preventative healthcare and lifestyle advice
- Support patients with long-term conditions
- Provide appropriate patient education and self-management advice
- Maintain accurate and comprehensive clinical records
- Participate in clinical audits and quality improvement
- Work closely with GPs and the wider multidisciplinary team
About You
We are looking for a confident and patient-focused Practice Nurse who is comfortable working autonomously while also being a strong team player.
Essential
- Registered Nurse with current NMC registration
- Previous Practice Nurse or primary care experience
- Experience in chronic disease management
- Strong clinical assessment skills
- Excellent communication and interpersonal skills
- Ability to work independently
- Commitment to providing high-quality patient care
Desirable
- Cervical cytology qualification
- Immunisation and vaccination experience
- Asthma and COPD experience
- Diabetes management experience
- Experience using EMIS or SystmOne
- Additional clinical qualifications or specialist interests
