Care Coordinator to join our growing office team.
Key Responsibilities
- Coordinate domiciliary and complex care packages.
- Monitor ECM for missed visits, late visits, missed task reporting and medication recording.
- Investigate GPS anomalies and suspected fraudulent log-ins.
- Monitor operational communications and action Field Supervisor feedback.
- Allocate and monitor actions through to completion.
- Support safeguarding, quality assurance and CQC compliance.
- participation in the out of hours rota.
About You
- Previous Care Coordinator experience.
- Knowledge of complex care.
- Experience with ECM and digital care systems.
- Excellent communication and organisational skills.
- Full UK driving licence.
