Working with our community nursing team, you’ll coordinate and deliver personalised care that helps people stay well and remain independent at home. You’ll play a key role in supporting patients with complex health needs, long term conditions and palliative care, helping to improve outcomes and reduce avoidable hospital admissions.
As a Case Manager, you will manage a defined caseload and work closely with patients, carers, General Practitioners, community services and other healthcare professionals. You will use your clinical expertise to assess needs, develop care plans and provide high-quality nursing interventions, while supporting colleagues and promoting best practice.
What you’ll do:
Assess, plan, deliver and review care for patients with complex nursing needs
Coordinate care and work with partners across health and care services
Provide specialist nursing interventions and support patient self-management
Manage and prioritise a caseload while maintaining accurate electronic care records
Support and develop colleagues through supervision, guidance and shared learning
What we’re looking for:
Registered Nurse qualification with current professional registration
Experience caring for people with long term conditions and complex health needs
Strong assessment, communication and care coordination skills
Ability to work independently and as part of a multidisciplinary team
Commitment to delivering safe, person-centred, evidence-based care and continuous professional development
