CLINICAL LEAD AND CARE COORDINATOR

Working pattern: Monday to Friday, with participation in the management on-call rota
Lumen Healthcare provides specialist domiciliary homecare, complex care, live-in care and nursing support to adults in their own homes.
We are recruiting an experienced Registered Nurse to work as our combined Clinical Lead and Care Coordinator. The successful candidate will lead clinical assessments, care planning, package mobilisation, staff competency and ongoing clinical oversight.
Main responsibilities
- Complete initial and ongoing clinical assessments.
- Develop person-centred clinical care plans and risk assessments.
- Assess the suitability and clinical requirements of new care packages.
- Coordinate nursing and complex-care teams.
- Review care plans following changes in health, hospital admission, incidents or deterioration.
- Complete medication, respiratory, nutrition, epilepsy, skin-integrity and emergency assessments.
- Provide clinical supervision to registered nurses and complex-care workers.
- Assess and document staff competence for person-specific clinical tasks.
- Ensure clinical tasks are only delegated to appropriately trained and competent staff.
- Review incidents, medication errors, safeguarding concerns and clinical deterioration.
- Liaise with GPs, consultants, hospitals, commissioners, therapists and community teams.
- Support hospital discharge planning and package mobilisation.
- Participate in audits, governance meetings and quality-improvement activity.
- Contribute to the management on-call and emergency escalation arrangements.
Essential requirements
- Current NMC registration as a Registered Nurse.
- At least three years’ experience in ICU, complex care, community nursing or a comparable clinical environment.
- Practical experience of tracheostomy care, ventilation, suction, oxygen therapy and respiratory deterioration.
- Experience in enteral feeding, epilepsy care, rescue medication and complex medication management.
- Experience completing clinical assessments, care plans and risk assessments.
- Experience supervising staff and assessing clinical competence.
- Knowledge of safeguarding, consent, the Mental Capacity Act and least-restrictive practice.
- Experience supporting autistic people or people with a learning disability.
- Strong written, digital and verbal communication skills.
- Full driving licence and access to suitable transport, subject to reasonable adjustments.
Desirable requirements
- Clinical leadership or care-coordination experience.
- Community complex-care experience.
- Experience of diabetes, insulin, bowel care, catheter care, stoma care or complex wound care.
- Practice assessor, clinical educator or recognised teaching qualification.
- Leadership and management qualification.
Appointment is subject to satisfactory identity, employment-history, enhanced DBS, Adults’ Barred List, professional-registration and reference checks.
Pay: £30,000.00-£45,000.00 per year
