Job Purpose
Support our client within his home and in the community, encouraging independence.
As a Support Worker you will be responsible for:
- Providing care in accordance with the current best practices, according to policy and procedures, agreed standards, legislative requirements, relevant regulations under the direction of the Case Manager
- Maintaining skills at a current level and undertaking such training and development as may from time-to-time be required to maintain practices as up-to-date.
As a Support Worker you will always:
- Ensure the client is at the heart of the care delivery and their wishes and preferences enhance their wellbeing
- Assume responsibility for promoting and safeguarding the welfare of the client
- Develop effective working relationships with other employees within MSPCM
- Work in cooperation with members of the multi-disciplinary team (therapists) to maximize opportunities for the client
- Attend statutory training and any other training as directed by the management
Specific Support Worker Skills and Duties:
- Maintain accurate and detailed handover (daily) records using an electronic app (Log My Care)
- Ensuring all documents are kept up to date in accordance with MSPCM policy
- Ensuring the prompt and accurate logging of any Accident/Incidents/Near Misses
- Build rapport with the client by establishing personal connections and showing interest in their life
- Effectively communicating with the multi-disciplinary team, with the client and their family/friends, and within the care support team
- Accurately complete stock checks for P.P.E and monitor medical supplies and, if running low, notify relevant persons
- Recognise people’s needs for alternative methods of communication and responding accordingly
- Attend all meetings called by the client’s parents and Case Manager
- To work with client’s family and friends. This may involve the highest degree of sensitivity and insight into family dynamics.
- Communicate any problems, concerns or changes to the client’s family members as needed
- Ensuring Accident/Incidents/Near Misses are escalated to the relevant persons
- Support Workers need to be able to adapt and address situations quickly. Plan, develop, implement and assess approaches to promote health and well-being whilst recognising and reporting situations where there might be a need for protection.
- Supporting the client in all aspects of daily living and personal care including: bathing, toileting/continence, eating/ feeding, dressing, grooming, play time with a variety of stimulating toys and equipment.
- You will be expected to assist with domestic chores at times including: keeping the client’s rooms tidy, clean and hoovered. Changing bed linen, washing and ironing, cleaning of client’s equipment.
- Move and handle as recommended in the moving and handling training (safely using hoisting equipment) and as per MSPCM policy and procedure. Bespoke training will be available for all transfers into the clients’ equipment.
- Enable the client to live as full, active and comfortable life as possible, planning and arranging social and recreational activities within the home and in the local community, motivating and encouraging participation in such, taking into account the client’s health and medical condition.
- Provide stimulation and opportunities for the client, for interactive/constructive interaction, in a variety of ways and settings.
- Provide assistance, support, guidance and encouragement as directed by the Case Manager, parents and therapists. ensure the individual needs of the client are met at all times.
- Reinforce and assist in any therapy programmes, including Occupational Therapy/Physiotherapy/Speech and Language Therapy, Assistive Technology, and Educational Psychology programmes under the guidance of the necessary professionals
- Travelling with, act as driver, or take responsibility for the client when travelling, ensuring that safety is a priority when accompanying the client on each outing.
- Ensure client confidentiality in record keeping and discussion concerning the client
- Ensure dignity, respect and privacy is always maintained for the client and their family
- Undertaking any other appropriate and reasonable task in relation to the employment as agreed with the client’s family and Case Manager.
Special Requirements
- Being flexible in hours of work to satisfy the client and their family’s needs.
- There may be times when you will need to go on holiday with the client and her family in the U.K and abroad.
