We have a great opportunity for people with previous experience within domiciliary support, to advance their career by progressing to work at a supervisory level as part of the management team.
We are looking for a Coordinator to work as part of our team in the West of Conwy, delivering domiciliary services to adults.
You will support the Team Manager by co-ordinating the work of community support staff in an effective and efficient way, for example the rostering of work, undertaking staff supervision, completing manual handling assessments and completing outcome focussed plans with individuals and their representatives.
You will be coordinating a team of staff who provide a flexible approach and support individuals and their carers in their own homes or other community settings to maximise their independence and improve health and well-being in a coordinated way.
Excellent communication and literacy skills plus the ability to work with a range of IT systems, are essential for the post.
There will be various training opportunities to enable you to fulfil the role as part of a multi-disciplinary team, for example achieving the BTEC in the provision of small aids and equipment, management training, training to support the delivery of Outcome Focussed Care and Support as defined in the Social Care and Well Being Act.
Ability to converse in Welsh with older people about their health and wellbeing is desirable. The ability to write in English is essential and desirable in Welsh
The community support team delivers services 365 days of the year. We are therefore looking for individuals who have a flexible outlook, and able to work unsociable hours on occasion, to meet the demands of the service.
