We are looking for a motivated and enthusiastic Physiotherapist to join the Home First Service within Doncaster. As part of our integrated community care offer, we offer a community physiotherapy service alongside partners to support clients in their own home or residency. The current post would be based across Community Physiotherapy, Urgent
Community Response and Enhanced Care Home Team, however, cover across the Home First Rehabilitation Service may be required at varying times.
The Home First Service provide assessment, treatment, and care support to patients in dedicated bed-based units, in their own homes or residency. The service aims to avoid acute hospital admissions and support early discharge, providing care in the community when possible. The Urgent Community Response service is an integrated, multi-agency pathway designed to prevent hospital admission. Community Physiotherapy involves assessment of patients in their own home or care home, for provision of individualised care plans and rehabilitation promoting independence. The post holder will be required to develop skills that will support all areas within the Home First Rehabilitation pathways, which
include:
- Community Physiotherapy
- Urgent Community Response
- Enhanced Care Home Team
- Community Rehabilitation Team
- Virtual ward
- Intermediate Care Wards
The role is for Band 6 post, 37.5 hours per week, flexible working will be considered. The service is 7 days a week with bank shifts currently available as part of this role to support weekend service cover.
The successful candidate will be required to have excellent communication skills. They will be capable of working autonomously as well as supporting the multidisciplinary team in assessment and development of a holistic management plan. The role requires organisation and ability to prioritise your own workload balancing the demands of the role.
The Trust provides regular clinical supervision with a Personal Development Review. There is a range of Continuing Professional Development opportunities including in-service training, access to external courses, and a post graduate training support. We facilitate
student education and have close working relationships with neighbouring teams in both Health and Social Care partners.
You should be HCPC registered, have appropriate post qualification experience of working within rehabilitation, and be able drive/ travel to various locations over the geographical area of Doncaster.
