We are seeking a dedicated and experienced Care Manager to oversee the delivery of high-quality care within assisted residential care settings. The successful candidate will be responsible for managing care plans, risk assessment, supervising staff, and ensuring the well-being of residents, particularly those requiring specialised dementia and other mentals care. Duties
- Develop, implement, and regularly review personalised care plans tailored to individual needs, including dementia and Huntington's Disease..
- Supervise and support care staff, providing leadership and guidance to ensure high standards of service delivery.
- Oversee medication administration in accordance with regulatory requirements and best practices.
- Coordinate with healthcare professionals to ensure comprehensive health management for residents.
- Monitor resident progress and adjust care strategies as necessary to optimise well-being.
- Ensure compliance with all legal, safety, and organisational policies related to senior care and care home operations.
- Lead staff training programmes focused on dementia care, medication management, and person-centred approaches.
- Maintain accurate documentation of care activities and resident records in line with regulatory standards.
- Foster a warm, respectful environment that promotes dignity, independence, and quality of life for residents.
Skills
- Proven experience in assisted living or nursing home environments, with a strong background in senior care.
- Extensive knowledge of dementia and Huntington's Disease disease management.
- Demonstrated leadership skills with previous supervising or management experience within a care setting.
- Excellent organisational skills with the ability to develop effective care plans and manage multiple priorities.
- Competence in medication administration adhering to safety protocols.
- Compassionate approach combined with strong communication skills to liaise effectively with residents, families, and healthcare professionals.
- Ability to lead by example, motivate teams, and foster a positive working environment.
- Knowledge of relevant regulations governing eldercare services in the UK.
