At Home Straight, we are a premium, CQC-registered domiciliary care provider dedicated to delivering exceptional hourly and live-in care, predominantly to older adults. As we continue to grow, we are looking for a skilled, empathetic, and professional Care Assessor to join our specialist team and act as the primary ambassador for our service.
The Role
As a Care Assessor, you are the crucial first link between us and a new client. You will be the "first face" a family sees, making this role equal parts clinical/compliance excellence and brand ambassadorship.
You will visit prospective and existing clients in their homes to conduct thorough, person-centered care and risk assessments, elegantly showcasing the value of our service. Working hand-in-hand with our office team, you will ensure a seamless, safe, and welcoming onboarding experience, as well as maintaining up-to-date, compliant care documentation for our existing client base.
Key Responsibilities1. New Client Assessments & Business Growth
- Act as the primary point of contact for new care inquiries, visiting prospective clients in their homes to showcase our hourly visiting service.
- Establish immediate trust and rapport with families by expertly articulating our agency's value, ensuring they feel comfortable and confident in our care.
- Conduct comprehensive initial holistic assessments to determine care needs, lifestyle preferences, and desired outcomes.
2. Care Planning & Compliance (CQC Standards)
- Write detailed, bespoke, and person-centered care plans that meet and exceed Care Quality Commission (CQC) regulations.
- Formulate robust risk assessments (including environmental, mobility, and safeguarding risks) to ensure the safety of both the client and our care staff.
- Design and implement safe medication management plans in line with regulatory frameworks and best practices.
3. Client Onboarding & Collaboration
- Work closely with the office team, care coordinators, and registered manager to ensure a smooth transition from initial assessment to the first day of care delivery.
- Provide clear briefings to the care delivery team regarding new clients' specific needs and nuances.
4. Reviews & Quality Assurance
- Regularly reassess existing clients, proactively updating care plans and risk assessments to reflect changing needs.
- Respond swiftly to changes in a client’s health condition or post-hospital discharge to modify care packages accordingly.
5. Business Support & Development
- Support other operational aspects of the business as required to maintain service excellence.
- Participate in the continuous training and professional development provided by our wider team of industry experts.
Candidate Requirements
Experience & Qualifications
- Proven Experience: Demonstrated experience in writing detailed, compliant care plans and conducting risk assessments within a domiciliary care setting.
- Sector Knowledge: A strong understanding of CQC fundamental standards, safeguarding procedures, and safe medication management.
- Qualifications: NVQ/QCF Level 3 in Health and Social Care (or equivalent experience) is highly desirable.
Skills & Attributes
- Exceptional interpersonal and communication skills; someone who is warm, professional, and naturally persuasive.
- Excellent written English with the ability to draft accurate, respectful, and legally robust care documentation.
- A team player who thrives on working alongside an office support network.
- A full UK driving license and access to a reliable vehicle for home visits is essential.

Pay: £30,000.00-£32,000.00 per year
