Applications are invited for an enthusiastic Clinical Nurse Specialist to work within the Anticoagulant team within Northumbria Healthcare NHS Foundation Trust. The focus of this role is to participate in the provision of a safe and co-ordinate anticoagulant service across North Tyneside and Northumberland. You will be part of a team of nurses and support staff undertaking anticoagulant monitoring for the introduction and maintenance of patients on anticoagulation providing education, advice and support to outpatients and inpatients. Acting as a resource to other professionals across primary and secondary care is a key element of this post.

Providing safe, effective and co-ordinated care within anticoagulation service covering North Tyneside and Northumberland, this will include -
Undertaking INR monitoring clinics within the hospital and community setting see both new and follow up patients
Undertaking initiation of anticoagulation clinics
Undertaking monitoring of in-patients on anticoagulants in all wards
Advising and education of patients and staff on all anticoagulants

The anticoagulant service at Northumbria Healthcare provides care for approximately 1800 patients, with approximately 26,000 attendances per annum.
A senior nurse working to provide a safe, effective and co-ordinated anticoagulant service for North Tyneside and Northumberland in the community and acute sectors.
Providing an accessible specialist anticoagulant advice service for primary and secondary care including advice on diagnosis and management of prevention of venous thromboembolism.
Developing and providing educational support for all grades of staff from a variety of professions as well as for patients and their carers.
Provides highly specialist clinical assessment, planning, implementation and evaluation for a high number of patients receiving anticoagulation. This includes the initial assessment, completion and interpretation of tests, care and education of patients newly commenced on anticoagulation.
Assessment of individuals’ suitability for anticoagulation and for signs of underlying disease requiring referral to other clinicians is a major part of this role.
Provides medication to patients without medical prescription through patient group directions.
Provides highly specialist advice, concerning all aspects of anticoagulant care and VTE diagnosis and management to clients/carers and all staff within primary and secondary care through to consultant/GP level.
Responsible for advising doses of warfarin and management of anticoagulation in situations where anticoagulation may be unstable or need to be managed according to particular situations e.g. during the pre and post operative period, in the presence of drug concordance issues, drug interactions, concurrent illness and interacting lifestyle factors. Responsibility is increased due to the fact that interaction may be via the telephone/a third person, and in the absence of all the relevant information required to inform clinical management decisions.
Management of patients who are significantly under or over anticoagulated. This includes assessing risk factors for haemorrhage and arranging reversal of warfarin in dangerously over anticoagulated patients.
Ensuring accurate and up to date records are maintained.
Physical skills including venepuncture, capillary and point of care blood testing, administration of subcutaneous injections.
