SureCare Calderdale & Kirklees is an award winning home care provider supporting people to remain safe, independent and comfortable in their own homes.
We are looking for an experienced and organised Care Co-ordinator to join our office team in Halifax.
This role would suit someone with previous domiciliary care experience who is confident managing rotas, responding to changes, supporting care workers and communicating with service users and their families.
The Role
As a Care Co-ordinator, you will be responsible for helping to ensure the safe, effective and consistent delivery of care services across Calderdale and Kirklees.
You will work closely with the Registered Manager, Care Manager, office team and care workers to ensure care visits are fully covered and delivered in line with service users’ assessed needs.
Your duties will include:
- Creating, managing and updating staff rotas.
- Ensuring all care visits are allocated and safely covered.
- Responding to sickness, emergencies and last-minute changes.
- Matching care workers with service users based on skills, experience and preferences.
- Monitoring continuity of care.
- Coordinating new and existing care packages.
- Maintaining accurate electronic care plans, schedules and records.
- Communicating with service users, families, care workers and healthcare professionals.
- Supporting the recruitment and onboarding of new care workers.
- Screening applications and conducting telephone and face to face interviews.
- Completing references, Right to Work checks and DBS applications.
- Coordinating staff inductions and mandatory training.
- Supporting care workers through the Care Certificate.
- Maintaining training and compliance records.
- Completing spot checks, observations and competency assessments.
- Auditing care records and medication documentation.
- Supporting investigations into incidents, accidents and medication errors.
- Assisting with safeguarding concerns and enquiries.
- Supporting CQC inspections and Local Authority monitoring visits.
- Participating in the out-of-hours on-call rota.
Essential Requirements
To be considered for this role, you must have:
- Previous experience working within domiciliary care or health and social care.
- Experience coordinating care visits, managing rotas or scheduling staff.
- A good understanding of CQC regulations, safeguarding and person centred care.
- Strong organisational and time management skills.
- Excellent communication and interpersonal skills.
- The ability to manage competing priorities and work well under pressure.
- Good IT skills, including Microsoft Office.
- A professional, reliable and approachable manner.
- A full UK driving licence.
- Access to a reliable vehicle insured for business use.
- The flexibility to travel throughout Calderdale and Kirklees.
Desirable Experience
The following would be beneficial but is not essential:
- Level 3 Diploma in Adult Care or an equivalent qualification.
- Experience using electronic care planning or rostering systems.
- Experience recruiting and interviewing care workers.
- Experience delivering inductions or staff training.
- Experience completing spot checks and competency assessments.
- Knowledge of Local Authority commissioned care services.

Additional Information
This role requires regular travel throughout Calderdale and Kirklees to complete service user visits, spot checks, care worker observations, recruitment activities and emergency cover.
A full UK driving licence and access to a reliable vehicle insured for business use are essential requirements.
Applicants must also be willing to participate in the out of hours on-call rota.
Pay: £27,560.00 per year
