Job Overview
We are seeking a dedicated and organised Care Coordinator to join our team. This vital role involves overseeing the delivery of high-quality care within care homes specialising in senior care. The successful candidate will be responsible for coordinating care plans, supervising staff, and ensuring that residents' individual needs are met with compassion and professionalism. A strong emphasis is placed on excellent communication skills, leadership abilities, and the capacity to manage multiple responsibilities efficiently. The Care Coordinator will play a key role in fostering a positive environment for residents and staff alike.
Duties
- Develop, implement, and review personalised care plans tailored to each resident’s needs
- Supervise and support care staff to ensure high standards of service delivery
- Coordinate daily activities and schedules to optimise resident wellbeing and comfort
- Liaise with healthcare professionals, families, and external agencies to facilitate comprehensive care
- Monitor compliance with health and safety regulations within the care environment
- Organise training sessions and professional development opportunities for team members
- Maintain accurate records using IT systems and ensure documentation is up-to-date
- Oversee medication administration procedures in accordance with prescribed protocols
- Lead by example in promoting a caring, respectful, and professional atmosphere within the care home
Qualifications
- Proven experience in senior care or similar roles within a care home setting
- Supervising experience with demonstrated leadership qualities
- Strong communication skills, both verbal and written, with the ability to liaise effectively with residents, families, and multidisciplinary teams
- Valid driving licence and ability to travel between locations as required
- Competent IT skills for managing electronic care plans and documentation
- Knowledge of care planning processes and regulatory standards in senior care environments
- Ability to organise workload efficiently whilst maintaining attention to detail
