Frailty Nurse / Frailty Paramedic Prescriber

Salary: £48,000 to £55,000 per annum depending on experience and qualifications
Hours: Full time or part time hours considered
Contract: 12 months fixed term, with possibility of permanent
The Primary Care Alliance CIC is a local primary care provider working across Berkshire West. We support general practice and deliver services at scale, working closely with practices, PCNs, Royal Berkshire NHS Foundation Trust, Berkshire Healthcare Foundation Trust, the ICB and wider system partners.
Our service portfolio is growing and currently includes the Urgent Care Centre at the Royal Berkshire Hospital, Lipid Optimisation, the emerging Women’s Health Hub, and wider hub-based services across Reading, Wokingham and West Berkshire.
The Proactive Neighbourhood Care and Remote Monitoring service will support people living with frailty, complex needs and those who are housebound. The service will combine proactive home visiting, care coordination, remote monitoring and links into wider neighbourhood and Local Authority support.
The Frailty Prescriber will provide autonomous clinical assessment and treatment for people living with frailty and complex needs, mainly in their own homes. The role will combine proactive clinical review, medicines optimisation, care planning, remote monitoring escalation and close working with general practice and wider neighbourhood teams.
Main duties will include
- Undertake comprehensive and proportionate clinical assessments in patients’ homes and remotely.
- Assess, diagnose, prescribe and manage treatment within professional competence and agreed clinical pathways.
- Identify deterioration, falls risk, medicines issues, unmet long-term condition needs and wider social concerns.
- Develop and update personalised care plans, including escalation and anticipatory planning where appropriate.
- Review remote monitoring alerts and make timely clinical decisions, including home visits or onward referral.
- Work closely with GPs, the HCA, care coordinator, social workers, social prescribers and other community services.
- Maintain accurate contemporaneous records on EMIS and relevant digital systems.
- Contribute to clinical audit, quality improvement, incident learning and service development.
- Provide advice and support to less experienced team members within scope of practice.
We are looking for someone with:
- Current NMC or HCPC registration - Essential
- Independent prescribing qualification - Essential
- Advanced clinical assessment and autonomous decision-making experience - Essential
- Experience supporting people living with frailty, complex needs or multiple long-term conditions - Essential
- Experience of community or home visiting work - Essential
- Full UK driving licence and access to a suitable vehicle for work - Essential
- Advanced practice qualification at Level 7 or equivalent experience - Desirable
- Experience in primary care, urgent community response, virtual wards or remote monitoring - Desirable
- Frailty, care of the older person, falls or medicines optimisation training - Desirable
- Experience in primary care, urgent care, GP federations, hub-based services or community healthcare would be a strong advantage.

- The successful candidate must be able to travel between operational sites across Berkshire West.
Pay: £48,000.00-£55,000.00 per year
