We are looking for a passionate, motivated individual to join our Move 2 Independence (M2I) service in West Somerset & Sedgemoor for 7 hours per week.
This role involves helping people rebuild their strength, confidence, and independence after a hospital stay. This is a rewarding role where you will see the direct impact of your work, supporting people to stay active, live well, and remain independent at home.
As an Activator, you will deliver one to one movement sessions in participants’ homes, supporting them to regain confidence in their movement and reduce the risk of hospital readmission.
You will manage your own caseload and take a person centred approach, adapting sessions to individual needs and, where appropriate, involving family members or support networks to encourage long-term independence.
The West Somerset & Sedgemoor role specifically includes supporting participants who have experienced a stroke, alongside those with a range of other medical conditions, ensuring safe and effective support in every session.
Applicants should:
- Hold a minimum of a Level 2 Fitness Instructor qualification (or equivalent)
- Be confident delivering individualised physical activity sessions
- Understand common referred conditions, including stroke, falls, neurological conditions, musculoskeletal conditions, and mental health, to support participants safely and effectively
- Be able to work in a person centred, supportive way
- Have access to independent transport to travel locally
- Understand IT programmes to track participant progress and be confident speaking with health professionals and members of the public
Owing to the nature of this position, any offer of employment will be subject to a satisfactory Enhanced DBS check.
