Responsibilities

- Communicate effectively with clients, families, and healthcare professionals to ensure seamless coordination of care and address any concerns promptly.
- Maintain accurate records of care activities, medication administration, and any changes in client condition using IT systems designed for healthcare documentation.
- Support clients in engaging with social activities and community programs to promote mental and emotional well-being.
- Follow health and safety protocols diligently to ensure a clean, safe environment for clients and staff alike.
- Collaborate with the healthcare team to monitor client progress, report any issues or changes in condition, and contribute to ongoing care planning.
Requirements
- Previous experience or training in nursing or healthcare support is highly desirable but not mandatory; enthusiasm for learning is essential.
- Ability to develop and follow detailed care plans tailored to individual needs.
- Excellent communication skills to interact effectively with clients, families, and multidisciplinary teams.
- Proficiency with IT systems used for documentation and communication within healthcare settings.

Pay: £1,900.00-£30,000.00 per month
