Job Overview
We are seeking a dedicated and compassionate Care Home Assistant Practitioner to join our team. This role involves providing high-quality support and care to residents within a care home environment, ensuring their wellbeing, safety, and comfort. The ideal candidate will possess strong leadership skills, excellent communication abilities, and relevant experience in senior care. This role also requires the ideal candidate to have relevant clinical skills to support our Nursing team
Responsibilities
- Assist with daily personal care routines for residents, including bathing, dressing, and mobility support
- Implement and follow individualised care plans
- Supervise and support carer team , fostering a positive team environment and ensuring high standards of care
- Communicate effectively with residents, families, and multidisciplinary teams to coordinate care and address needs
- Maintain accurate documentation using IT systems( PCS Software) for care records and progress notes
- Support residents with medication administration as per prescribed guidelines
- Ensure the safety and wellbeing of all residents by adhering to health and safety policies and procedures
- Clinical duties - Monitoring Residents health , check vital signs and manage blood sugar levels
- Assist Nurses with treatments including Catheter care/ Wound management
- Support Nursing team/ Senior team with delivering compassionate , respectful end of life care to Residents
Qualifications
- Experience in senior or elderly care within a care home setting is required
- Supervising or leadership experience in a healthcare environment is advantageous
- Knowledge of care plans, medication management, and IT systems utilised in healthcare settings
- Strong communication skills to effectively liaise with residents, families, and colleagues
- Valid driving licence to assist in hospital/home assessments for potential clients
- Demonstrated ability to supervise team members with professionalism and empathy
- Ability to work flexibly across various shifts as required by the role
- Required ;- The Care Certificate and Level 3 Diploma in Health and Social Care
- Required ;- Certified training in performing specific clinical procedures in blood works/catheter care and managing wounds
- Required ;- Certified Level 3 Safe handling of medicines

Pay: £27,144.00-£29,390.40 per year
