Service Coordinator
Location: London (On-site)
Salary: £25,541 to £27,200 per annum, pro rata (plus ILW, if residing & working in London)
Hours: 35 per week (Mon-Fri, 9am-5pm)
Contract: Fixed term contract (until 31st March 2027 with possible extension subject to funding)

As a Service Coordinator, you'll play a key role in helping people make a safe and successful transition from hospital to home. Leading a dedicated team of staff and volunteers, you'll ensure vulnerable people receive the practical and emotional support they need to regain confidence, maintain their independence, and avoid unnecessary hospital readmission. Working closely with hospitals, health and social care professionals, and community partners, you'll coordinate a service that delivers real impact when people need it most.
This is a highly visible, community-based role involving regular travel across across hospitals and partner organisations within the scheme's coverage area across London to meet service needs. You will also be expected to provide direct frontline support to service users when required, ensuring continuity of service during periods of staff absence or increased demand. What will a day in the life of a Service Coordinator involve?
- Acting as the main point of contact for hospital and community referrals.
- Coordinating, supporting and motivating a team of staff and volunteers to deliver safe and effective services.
- Building and maintaining relationships with hospitals, GP surgeries, community services and other external partners, while actively promoting the service through ward rounds, engagement with healthcare teams, presentations, and local networking activities.
- Monitoring service delivery, maintaining accurate records and ensuring quality, safeguarding and health and safety standards are met.
- Attending stakeholder meetings, reviewing service outcomes and supporting service development.
- Providing frontline support when needed, helping service users transition safely home from hospital, assisting with practical tasks and offering reassurance and emotional support throughout their recovery journey.
- Travelling regularly across London (scheme-specific locations) and supporting neighbouring services where required to meet operational needs.
To be a successful Service Coordinator, what will you need?
- Experience managing or supervising people and working in a team environment.
- Strong communication and relationship-building skills, with the ability to engage confidently with health and social care professionals and other stakeholders.
- Experience coordinating competing priorities and responding effectively in a fast-paced environment.
- Confidence using IT systems and maintaining accurate service records.
- An understanding of safeguarding responsibilities and a commitment to ensuring the safety and wellbeing of vulnerable people.
- A willingness to travel extensively across London, adapting to changing service requirements.
- The ability to undertake practical frontline activities when required, including carrying shopping or equipment and supporting service users in the community.

Please note: This is an active, community-based role involving significant travel across London. The successful candidate must be comfortable travelling regularly between locations and undertaking occasional practical support duties to ensure continuity of service delivery.
