Community-based all around Hertfordshire (service users’ homes and local community settings)
Job Purpose
To provide high-quality, person-centred care and support to individuals living in their own homes and within the community, including people with mental health needs, learning disabilities, autism, physical disabilities, and other support requirements. The role aims to promote independence, dignity, wellbeing, and social inclusion in line with individual care and support plans.
Responsibilities:
Personal Care & Daily Living Support
- Provide personal care support, including washing, dressing, toileting, continence care, and mobility assistance, in a respectful and dignified manner.
- Support service users with meal preparation, nutrition, hydration, and medication prompting or administration (where trained and authorised).
- Assist with household tasks such as light cleaning, laundry, and shopping to maintain a safe living environment.
Mental Health & Emotional Support
- Provide practical and emotional support to individuals experiencing mental health challenges, promoting recovery and resilience. Encourage coping strategies, daily routines, and positive mental wellbeing.
- Support service users to attend appointments, engage with community activities, and reduce social isolation.
- Observe, monitor, and accurately record changes in mental health or behaviour, escalating concerns appropriately.
Person-Centred Support
- Follow individual care and support plans, risk assessments, and guidance from healthcare professionals.
- Promote choice, independence, dignity, and respect at all times.
- Encourage service users to develop and maintain life skills and independence.

- Safeguard vulnerable adults by recognising and reporting signs of abuse, neglect, or exploitation in line with safeguarding policies.
- Maintain accurate, timely, and confidential records in line with GDPR and organisational procedures.
- Work in accordance with CQC Fundamental Standards, company policies, and relevant legislation.
- Maintain professional boundaries and act as a positive role model.
- Teamwork & Communication
- Work collaboratively with colleagues, managers, families, and external professionals (e.g. GPs, social workers, mental health teams).

Essential
- Caring, compassionate, and non-judgemental approach.
- Commitment to promoting dignity, independence, and wellbeing.
- Good communication and interpersonal skills.
- Ability to work independently and as part of a team.
- Willingness to work flexible hours, including evenings, weekends, and bank holidays.
- Basic literacy and IT skills for record keeping.
- Right to work in the UK.
Desirable
- Previous experience in domiciliary care, mental health support, or community support.
- NVQ/QCF Level 2 or 3 in Health & Social Care (or willingness to work towards).
- Experience supporting individuals with mental health needs, learning disabilities, or autism.
- Full UK driving licence and access to a vehicle.
Training & Development
- Full induction and mandatory training provided (including safeguarding, medication, mental health awareness).
- Ongoing professional development and opportunities to gain recognised qualifications.
- Regular supervision and support from management.
