Salary: £24,800 - £26,324 per annum
Hours: 37.5 hours per week
Contract: 12 months fixed term, with possibility of permanent
The Primary Care Alliance CIC is a local primary care provider working across Berkshire West. We support general practice and deliver services at scale, working closely with practices, PCNs, Royal Berkshire NHS Foundation Trust, Berkshire Healthcare Foundation Trust, the ICB and wider system partners.
Our service portfolio is growing and currently includes the Urgent Care Centre at the Royal Berkshire Hospital, Lipid Optimisation, the emerging Women’s Health Hub, and wider hub-based services across Reading, Wokingham and West Berkshire.
The Care Coordinator will be central to the day-to-day organisation of the service. The postholder will coordinate referrals, appointments, visits and actions, maintain oversight of the caseload and act as a key link between patients, practices, clinicians and partner organisations.
This is a hands-on hybrid role. The successful candidate will be expected to work across our operational sites, predominantly at our offices in Lower Earley, and occasionally throughout Wokingham and emerging West Berkshire hubs. This is not a remote role but flexibility will be considered.
Main duties will include
- Receive and process referrals, check information and ensure patients are added to the correct pathway.
- Contact patients and carers, explain the service and arrange assessments, visits and follow-up.
- Maintain accurate caseload and action trackers and make sure outstanding actions are followed through.
- Coordinate clinician and driver schedules and support efficient use of available capacity.
- Liaise with GP practices, Local Authority teams, community services and other partners.
- Support remote monitoring onboarding and follow-up alongside the HCA and clinical team.
- Enter and update information accurately on EMIS, spreadsheets and other service systems.
- Support activity, outcome and performance reporting and identify gaps or delays to the Service Manager.
- Handle patient information sensitively and escalate clinical or safeguarding concerns promptly.
We are looking for someone with
Experience in an administrative, care coordination, healthcare or patient-facing role - Essential
Excellent organisation, communication and follow-up skills - Essential
Confident using IT systems, email and spreadsheets - Essential
Ability to manage a busy workload and maintain accurate records - Essential
Ability to communicate sensitively with patients, carers and professionals - Essential
Experience in primary care, community services, frailty or remote monitoring - Desirable
Experience using EMIS or another clinical system - Desirable
Care Certificate, care navigation or relevant coordination training - Desirable
Experience in primary care, urgent care, GP federations, hub-based services or community healthcare would be a strong advantage.
