The Salford Adult Social Care (ASC) Therapy Team supports people across Salford—including children—to live as independently and safely as possible in their own homes. We provide person-centred assessments and interventions, enabling individuals to maintain independence and reduce risks within their home environment.
We are a forward-thinking and innovative service with a strong reputation for high-quality practice, particularly in moving and handling, specialist equipment, and complex housing adaptations. Many of our recommendations are funded through Disabled Facilities Grants (DFG), and our therapists have access to a wide range of equipment to support outcomes.
This is an exciting opportunity for a Band 6 Occupational Therapist to develop specialist community skills within a supportive and experienced team.
Screen and prioritise new referrals into the service
Carry out functional and holistic assessments within service users’ homes and, where required, in hospital settings to support safe discharge
Identify risks and provide practical, person-centred solutions to promote independence
Recommend and implement specialist equipment, moving and handling solutions, and complex housing adaptations
Complete DFG-related assessments and recommendations
Work collaboratively with multidisciplinary colleagues, families, and carers
