Background Information
Mr H is a 19‑year‑old young man who attends college and leads an active and enjoyable lifestyle. He has a keen interest in music, bowling, films, sensory‑based games, exercise, and values socialising with his family and friends. Supporting Mr H focuses on enabling him to participate fully in activities that are meaningful and enjoyable to him.
Mr H has a brain injury and severe developmental impairment, which impacts his cognitive, social, fine motor, and gross motor abilities. He requires dedicated, compassionate, and consistent support to meet his daily needs and to promote his independence, wellbeing, and quality of life.
The role of Support Worker involves providing structured, person‑centred support to Mr H throughout the day and night. You will work as part of a professional support team, following care plans and guidance from relevant professionals.
Mr H enjoys being active and following routine throughout the day, whereby he undertakes regular activities and enjoys spending time with his family and support workers and the successful candidate will be able to think creatively regarding making the daily tasks and activities engaging an meaning full ensuring Mr H continues to enjoy these an participates in these daily tasks.
Support Requirements
- 2:1 support during waking hours, primarily due to moving and handling needs, personal care, and support with daily activities
- 1:1 waking night support, ensuring Mr H’s safety, comfort, and wellbeing overnight
Key responsibilities
- Provide full personal care and physical support, ensuring dignity, comfort, and safety at all times
- Support with all activities of daily living, including positioning, mobility, and hygiene
- Assist with communication, recognising non-verbal cues such as vocalisations (e.g., grunting or high-pitched sounds when unhappy)
- Promote engagement in enjoyable activities such as:
- Sensory play
- Reading
- Watching television
- Music, songs, and rhymes
- Encourage social interaction
- Monitor emotional wellbeing and mood, using appropriate distraction and calming techniques where needed
- Maintain a warm, safe environment (e.g., keeping the room warm and avoiding open windows)
- Closely monitor health, particularly for early signs of chest infections (e.g., coughing), and report concerns immediately to the appropriate health authorities.
- Follow all care plans, risk assessments, and health protocols

Health & Safety
- Be vigilant regarding the individual’s susceptibility to chest infections and act promptly on any early symptoms
- Ensure all infection prevention and control measures are followed

About the Individual
- Limited verbal communication but expressive through sounds, repetitive words and statements and facial expressions
- Responds positively to music, songs, and familiar voices
- Enjoys sensory stimulation and calm, engaging activities
Skills & Experience Required
- Experience supporting individuals with complex needs (preferred)
- Understanding of neurological conditions
- Patient, caring, and attentive to detail
- Strong observational skills and ability to respond to non-verbal communication

Recording and reporting
Record and report all relevant information including:
- The care and support that you provide and assistance with medicines.
- Changes to condition or other concerns - you may be one of the few people your client sees regularly over a period of time; it is therefore essential to report any changes or causes for concern to your line manager promptly. You need to be clear about when to seek help and advice in order to keep client safe and promote their well-being.

As this employment is based in the private home, it is inevitable that on occasions the support workers will become privy to certain matters relating to the personal life and business affairs. These too should be treated with the utmost confidentiality.
