Job Summary
We are seeking an enthusiastic and motivated Practice Pharmacist to join our established and supportive multidisciplinary team. We would ideally welcome an Independent Prescribing Pharmacist, however applications from suitably experienced non-prescribing pharmacists will also be considered.
This is an exciting opportunity to play a key role in delivering high-quality patient care within a busy GP practice. You will work closely with GPs, nurses, healthcare assistants, care coordinators, physiotherapists, pharmacists and administrative colleagues to provide safe, effective, and patient-centred pharmaceutical care. As a Practice Pharmacist, you will be responsible for providing expert pharmaceutical services, supporting clinical decision-making, and ensuring safe medication use within the practice setting. Your role will be vital in enhancing patient outcomes through comprehensive medication management and collaborative healthcare delivery.
Responsibilities
As our Practice Pharmacist, you will work within your professional competence to deliver a range of patient-facing and medicine optimisation services, including:
- Running long-term condition medication review clinics (e.g. hypertension, diabetes, asthma/COPD, lipid management, AF and HF)
- Conducting structured medication reviews and comprehensive medicines optimisation reviews
- Supporting patients with polypharmacy and complex medication regimes
- Delivering falls prevention and medication-related risk reduction clinics
- Undertaking compliance and adherence reviews
- Responding to prescribing queries and medication-related requests
- Working collaboratively with GPs and other prescribers to provide a safe, robust and efficient repeat prescribing service
- Supporting quality improvement initiatives, clinical audits, and prescribing optimisation projects
- Providing medicines advice to patients and the wider practice team
- Dispense prescriptions accurately while ensuring compliance with legal and safety standards.
- Provide expert advice to patients regarding medication use, potential side effects, and interactions.
- Collaborate with healthcare professionals to develop and review treatment plans tailored to individual patient needs.
- Monitor patient progress and adjust medication regimens as necessary to optimize therapeutic outcomes.
- Educate patients on anatomy-related health issues and promote medication adherence strategies.
- Participate in clinical audits and quality assurance activities to uphold high standards of pharmacy practice.
- Stay informed about the latest developments in pharmacology to provide current best practices.
Qualifications
- MPharm degree or equivalent qualification
- Current registration with the General Pharmaceutical Council (GPhC)
- Independent Prescriber qualification recognised by the GPhC (for prescribing roles only)
- Minimum 6 months' experience working in a clinical environment
- Demonstrated experience in patient care settings, preferably within a clinical, community or hospital environment.
- Strong knowledge of human anatomy and pharmacology principles essential for effective medication management.
- Excellent communication and interpersonal skills to effectively educate patients and collaborate with healthcare teams.
- Ability to interpret medical charts, laboratory results, and other clinical data accurately.
- Confidence discussing medication-related issues with patients, clinicians, and external stakeholders

Desireable Criteria
- Experience working in General Practice
- Experience conducting Structured Medication Reviews (SMRs)
- Experience managing long-term conditions
- Understanding of QOF, medicines optimisation, and NHS prescribing initiatives
