Clinical oversight: Registered Nurses, Care Workers and Bank Workers assigned to care packages
The Care Coordinator / Clinical Lead / Registered Nurse coordinates Lumen Healthcare’s domiciliary, live-in and complex nursing care packages while providing professional clinical leadership.
The postholder will assess people’s needs, develop person-centred care plans and risk assessments, coordinate staffing, oversee medicines management, assess staff competence, monitor care delivery and respond to clinical concerns.
The role involves working with the Registered Manager, care staff, people receiving care, families, commissioners and external healthcare professionals. All duties must comply with the NMC Code, CQC regulations, relevant legislation, professional scope of practice and Lumen Healthcare policies.
Main duties and responsibilities
Care coordination
- Coordinate domiciliary, live-in and complex nursing care packages.
- Maintain accurate records of active, pending, suspended and closed packages.
- Ensure each package has an authorised assessment, care plan, risk assessment and staffing plan before care begins.
- Prepare safe rotas and match staff to people’s needs, preferences, risks and required competencies.
- Arrange cover for sickness, annual leave, training and unexpected absence, using bank workers where appropriate.
- Promote continuity of care.
- Monitor working hours, travel arrangements, rest periods and live-in care arrangements.
- Escalate unresolved staffing gaps to the Registered Manager.
- Communicate care arrangements to people, representatives, staff and commissioners.
Clinical assessment and care planning
- Complete comprehensive nursing and care assessments covering physical health, clinical needs, medicines, mobility, nutrition, communication, cognition, mental capacity and emotional wellbeing.
- Establish each person’s usual clinical presentation, risks and signs of deterioration.
- Determine whether Lumen Healthcare can safely meet the person’s needs and identify required staffing, skills, equipment and professional support.
- Prepare person-centred care plans, clinical protocols, risk assessments and emergency plans.
- Set person-specific escalation thresholds and record instructions for routine care, clinical procedures and emergencies.
- Obtain and record valid consent, apply the Mental Capacity Act 2005 where required and participate in best-interests decisions.
- Review documentation following incidents, hospital admissions, professional advice or changes in need.
- Ensure workers receive and understand current care information.
Complex clinical care
Provide, supervise or coordinate care involving:
- Tracheostomy care.
- Invasive or non-invasive ventilation.
- Oral and tracheal suction.
- Oxygen therapy.
- Enteral feeding.
- Epilepsy care.
- Diabetes care, blood-glucose monitoring and insulin administration.
- Catheter and stoma care.
- Complex bowel-care programmes.
- Wound care and pressure-area management.
- Clinical observations and deterioration monitoring.
- Pain and symptom management.
- Palliative and end-of-life care.
Clinical procedures may only be undertaken when authorised in the care plan and performed by workers assessed as competent.
Clinical delegation and competency assessment
- Make safe delegation decisions in accordance with the NMC Code.
- Confirm that delegated tasks are appropriate for the person, procedure and worker.
- Identify package-specific training requirements and assess competence through knowledge checks, simulation, supervised practice and direct observation.
- Maintain competency records and a clinical competency matrix.
- Confirm competence before unsupervised delegated practice.
- Set review dates and reassess competence following incidents, equipment or procedural changes, or concerns about practice.
- Suspend clinical authorisation where competence or safety is uncertain.
- Ensure training completion is not treated as evidence of practical competence without assessment.
Medicines management
- Assess medicine-support needs and establish the appropriate level of assistance.
- Prepare medicine risk assessments, care-plan instructions and medication administration records.
- Confirm that staff understand medicine instructions and escalation procedures.
- Monitor high-risk medicines and person-specific protocols.
- Review missed doses, errors, omissions and near misses, contacting prescribers, pharmacists or emergency services where required.
- Complete medicines audits and monitor improvement actions.
- Ensure medicines are ordered, received, stored, administered, recorded and disposed of safely.
Monitoring care delivery
- Monitor electronic call records during operational hours.
- Respond to missed, late, shortened or unconfirmed visits by contacting staff and people receiving care, arranging cover and assessing clinical risk.
- Review electronic care notes for changes, omissions or poor practice.
- Monitor nutrition, hydration, skin integrity, medicines and clinical observations where required.
- Identify deterioration, initiate appropriate escalation and record decisions, advice and actions.
Clinical leadership
- Provide clinical leadership, supervision and reflective-practice support to Registered Nurses and Care Workers.
- Review clinical performance, identify development needs and support induction, shadowing and supervised practice.
- Participate in recruitment and clinical interviews.
- Verify professional registration, qualifications and experience.
- Monitor NMC registration and revalidation requirements.
- Promote evidence-based, person-centred practice and challenge unsafe care.
- Address clinical performance concerns and refer employment or disciplinary matters to the Registered Manager.
- Support staff following incidents and emergencies.
Safeguarding and risk management
- Recognise and respond to abuse, neglect, exploitation and improper treatment.
- Follow safeguarding procedures, notify the Registered Manager or Safeguarding Lead and contact emergency services where there is immediate danger.
- Support referrals to the relevant local authority, preserve evidence and cooperate with enquiries.
- Review care plans and risk assessments following concerns.
- Promote least-restrictive, person-centred practice.
Incident management
- Respond to incidents, accidents, near misses and deterioration, ensuring immediate care and emergency action.
- Notify the Registered Manager and inform commissioners or external professionals where appropriate.
- Complete factual records and participate in investigations, root-cause analysis and duty of candour arrangements.
- Update care plans, risk assessments and staff instructions.
- Identify learning and monitor completion of improvement actions.
Quality assurance and governance
- Audit care plans, risk assessments, clinical records and medicines documentation.
- Complete clinical spot checks and observations of practice.
- Monitor missed visits, incidents, complaints, infections, hospital admissions and medicine errors.
- Maintain the clinical risk register and track care-plan reviews and competency expiry dates.
- Prepare clinical reports for governance meetings.
- Participate in CQC inspections and commissioner audits.
- Monitor policy compliance and contribute to service improvement using feedback from people, families, staff and professionals.
Infection prevention and equipment safety
- Assess infection risks and include appropriate controls in care plans.
- Monitor hand hygiene and personal protective equipment use.
- Confirm that clinical equipment is available, suitable and maintained, with current servicing records.
- Ensure workers are trained and competent to use assigned equipment.
- Report missing or defective equipment immediately.
Multidisciplinary working
- Work with GPs, hospitals, community and specialist nurses, pharmacists, therapy teams, Integrated Care Boards, local authorities and commissioners.
- Attend multidisciplinary meetings and professional reviews.
- Support safe hospital discharge and transfer of care.
- Obtain current clinical instructions, incorporate them into care plans and escalate unclear or conflicting advice.
On-call responsibilities
- Participate in agreed clinical or management on-call arrangements.
- Provide advice within professional competence, respond to urgent staffing and care concerns, and direct staff to emergency services where required.
- Maintain records of advice, decisions and actions.
- Escalate significant events to the Registered Manager.
- Receive a separate allowance where on-call duties fall outside contracted hours.
Person specification
Qualifications and professional registration
- Current NMC registration as a Registered Adult Nurse.
- Experience relevant to the people supported.
- Evidence of continuing professional development.
- Current mandatory and clinical training relevant to the role.
Experience
- At least three years of relevant post-registration nursing experience.
- Experience in complex care, an Intensive Care Unit or a comparable clinical setting.
- Experience completing nursing assessments, care plans and clinical risk assessments.
- Experience supporting people with complex or long-term health conditions.
- Experience recognising and responding to clinical deterioration.
- Experience supervising staff, assessing competence and coordinating care packages, rotas or clinical caseloads.
- Experience working with families, commissioners and multidisciplinary professionals.
- Experience maintaining accurate clinical records.

Practical and recruitment requirements
- Ability to work 25 hours per week, Monday to Friday, 9am to 2pm, with the ability to move to full-time hours.

It is desirable that the successful applicant has:
- A leadership or management qualification.
- A practice assessor, mentorship or clinical-teaching qualification.
- Experience in domiciliary, live-in or complex nursing care delivered in people’s homes.
- Current competence in tracheostomy care, ventilation, suction, oxygen therapy or cough-assist equipment.
- Experience with enteral feeding, epilepsy and prescribed rescue medicines, diabetes and insulin administration, catheter and stoma care, complex bowel care, wound care or pressure-area management.
- Experience supporting autistic people, people with learning disabilities, acquired brain injuries or neurological conditions.
- Experience providing palliative and end-of-life care.
- Oliver McGowan Mandatory Training at the appropriate tier.
- Experience using electronic care-planning, medication and rostering systems.
- Experience participating in CQC inspections, commissioner audits or clinical governance meetings.
A full UK driving licence and access to suitable transport.
