The Role
In this role, you will provide administrative, care coordination and social prescribing support across the Practice and Primary Care Network, ensuring patients receive timely, safe and effective care. The role requires close collaboration with patients, clinicians, the PPG, community services and external healthcare organisations.
Key Responsibilities
- Manage specialist vaccination and injection programmes, patient recalls, appointments and related clinical administration.
- Coordinate care for patients across a range of healthcare services, liaising with clinicians, community teams and external providers to ensure seamless care.
- Support frailty, palliative and end-of-life pathways, safety alerts, care planning and multidisciplinary working.
- Maintain carers’ services and support the Patient Participation Group (PPG), including attendance at one evening meeting per quarter. (6pm - 8pm)
- Deliver social prescribing support by assessing patient needs, signposting to appropriate community services and monitoring outcomes. Full social prescribing training will be offered.
What We're Looking For
- Previous work experience in a healthcare setting (essential)
- Previous work experience in a primary care setting (desirable)
- Previous social prescribing experience (desirable)
- Experience using Medicus Health (desirable)
- Strong communication and IT skills
- Ability to work in a team as well as autonomously
- Organised and adaptable
- Flexibility to work across our four Guildford locations
- GCSEs (or equivalent) in Maths and English
- Full time - 37.5 hours per week
