Responsibilities

- Provide transportation and driving support for appointments, errands, or outings as needed
- Monitor client health status, document progress, and communicate updates to healthcare teams or case managers
- Engage in light housekeeping tasks to maintain a safe and clean living environment
- Collaborate with healthcare professionals to ensure comprehensive patient care and adherence to medical protocols
Skills
- Proven experience in care home, assisted living, or home care environments with a focus
- Knowledge of medical terminology, medication administration, and basic nursing practices
- Certifications in first aid and CPR are highly desirable
- Strong understanding of behavior management techniques and case management principles
- Ability to perform heavy lifting safely when assisting clients with mobility or transfers
- Excellent communication skills to effectively interact with clients, families, and healthcare providers
- Valid driver’s license with reliable transportation for driving duties
