Working with a skilled Community Rehab team, you’ll support people in their own homes so they can live safely and independently.
You’ll join Occupational Therapists, Physiotherapists and Associate Practitioners who work closely together to deliver person-centred care.
This role makes a real difference to people with physical or neurological conditions, helping them regain confidence and independence. You’ll be supported with supervision from both disciplines and opportunities to build skills across Occupational Therapy and Physiotherapy.

What you’ll do:
- Carry out holistic assessments in people’s homes
- Agree goals and deliver personalised rehabilitation plans
- Support mobility and functional daily activities
- Recommend and support use of adaptive equipment
- Work with other services to ensure safe, effective care
What we’re looking for:
- Experience in a similar health or social care role
- Ability to manage a caseload with a good level of autonomy
- Strong communication and teamwork skills
- A flexible approach and willingness to learn across disciplines
- Ability to travel to support patients in their homes
